Social Services
Jolene Williams is RMC's Social Service Director. She provides programs and activities to help residents to meet their psychosocial goals and achieve the highest practical well being. Whether you are planning on returning to your home, or planning on transitioning to another facility after your discharge, the social service designee can help. The social service program was developed at this facility to help patients and residents attain or maintain the highest practicable physical, mental and psychosocial well being possible. There are many programs available to help you with your transition to home or to another facility such as an assisted living or nursing home type placement. There are also many referral services available for our patients and residents to help them with transition phases. The social service designee is responsible for helping the medical team steer your recovery and transition out of the hospital. Some of the tools the social service designee uses are a depression screening tool and a mini mental status exam. These shall be done during your hospital stay and shared with the Dr. to help determine your discharge decisions. The social services designee will also ask several questions to help with your transition. Upon your return home, the social service designee is availiable to help you to determine if there are any necessary services you have thought of since leaving our facility. Some of the services and referrals are available from the social service designee who visits you.
